Support Person Assistance Card

Fill out and print your application with an online printable form or a fillable PDF. Customers using screen reader software should use the online printable form. Applicants must complete Part A and Part B of the application. An authorized health care professional must complete and stamp Part C of the application. Incomplete forms will not be accepted.

Please note that the TTC will not reimburse any costs incurred to complete this application.

Mail completed application to the TTC with one (1) colour passport photo signed on the reverse by the authorized regulated health care professional who completes Part C of the Application. Applications can be mailed to:
